We ask that you bring the following items with you to your first appointment:
- Photo identification
- Insurance Cards
- Referral from your physician
- Any copies of medical testing e.g. Laboratory Results, Ultrasounds
- Your copayment/deductible is due at the time of your visit. We accept cash, checks, and all major credit cards.
- To to our forms page to access the forms that will be needed for your first appointment.
